We are revisiting the status of the new hospital being built by the Provincial Government of Abra. You may recall more than a year ago, the new hospital which is worth millions of pesos (through a bank loan) to built, suffered some structural damage due to rains. There was a big uproar by Abrenians around the world why such a very expensive building would suffer damage from just a slight of rain? Abrenians also questioned the alleged contractor Glen Eduarte, the current Tayum Mayor.
Governor Takit Bersamin responded through a news releases by PIA: Abra gov assures soundness of new hospital building
He gave his assurance that investigation is going to be conducted within their level because he is for good governance and transparency. He also appealed for the people not take verbal reports ‘because the damage has to be investigated to determine who is at fault and we have to consider many factors surrounding the circumstances.’
“I would like to tell everyone that I am determined to fix everything because I want to finish the project despite what happened. We need this new hospital very badly because as of now, our old hospital can no longer accommodate our sick patients,” he announced.
This news was in August 2011. That’s was more than a year ago. Abrenians are now wondering what has happen to his investigation. When will this hospital going to open? When will it benefit Abrenians who urgently need healthcare? We visited the new hospital and this is what we saw – an idle piece of building, a waste of taxpayers’ money! (abrenian.com photos)

Cleanliness must be maintained at all times.
Visitors should be limited for a couple of hours a day, a maximum number of visitors at a time. But one should be allowed at all times to be with the patient.
Emergency basic medicines/equipment must be available at all times.
Its good to have all the equipments like cat scan, etc…but what is needed most are the basic equipments and basic emergency medines.
The staff in all departments must do their job based on their job descriptions.
This is the hospital of all abrenian and every everyone is entitled to get the best medical care the hospital can offer. Patients must understand that they have resposibility too.
The staff are obliged to give the best medical care equally to all patients. Hospital staff are public servants but they need to begiven due respect as they are too, to the patients/relatives.
These are just a few suggestions i am making today.
I hope medical practioners outside Abra will vist the hospital and share their expertise to improve treatment of patients.

But look at them now, they travel anytime they like and even bought two houses in america worth 1 million dollars each. So these are where the loans he made for the hospital went.

Sad to say that the worth of the building is not that big. 40% of the money loaned was pocketed by the people in government,that including the governor. To my mind this is where the two houses in north hollywood worth 1 million each came from. With the exchange rate at 42 oesos to a dollar, 2 million dollars would be 84 million pesos. Take the 40% from 210 million and that would be 884 million. Wow what a coincidence. And there are ither projects loaned from the banks, that would explain his house in ayala alabang where the minimum price of a house is 50 million pesos.
I voted for him in 2007 in the hope that he would change the image of abra that including the health cars system of the province. But to my dismay the health cars system declined esoecially the abra provincial hospital. During the term of gov valera, the hspital was under the leadership of dra puruganan, who is incidentally the sister-in-law of the present caretaker of the aoh, dr gaza. Then the hospital was very clean, why? Beacause dra vicky was hands on in the cleanliness and management of the hospital. She had a free hand and had the support of the governor. But this time, dr gaza ia not free to implement whatever he would like to implement because if the meddling od “dra” ruby masamang bulsamin. Maybe dr gaza had the right intensions but could not resist the temptation of ruby. As they say, if you can’t beat them, join them.

There are many comments (19 responses) made on the NEW HOSPITAL .
In a Democratic form of Government, Presidential form like the USA or Parliamentary form of UK… there are two opposing parties. Check and balances are debated before an issue is settled.
In the USA… Democrats against Republicans. In the Philippines… Natinonalista against Liberal.
It was during this critical time the question of the contractor of the Hospital goes to…. contractor from outside the province or local talent contractor. Decision reached by argument won on the debate by the two opposing parties.

Some said the cost to built this hospital is in the 9 figures. Some called it a white elephant, some say it is a Ghostpital. Up to now, the hospital is not operational. Now, this group of dishonest individuals, suddenly became deaf, mute, and blind. Not a single person from this group came forward and say a word about the hospital. Their vigor and vitality suddenly waned.

One of each… computer ( lap-top or pc with Tower), android or smartphone, cell-phone).
On a regular days… the Secretary does the record keeping of the Medical History of the Community by entering informations on the Computer… vaccinations, births, deaths, who got sick and the dignosis attended by the Doctor or Nurse. Note: paper work is done away and the Recording via the Computer takes over.

1. TRIAGE MEDICAL CLINIC (TMC) located in each municipality.
2. ABRA PROVINCIAL HOSPITAL (APH) … at this point I proposed “ABRA MEDICAL MEMORIAL CENTER”
>>>AMMC I consider as the place where final treament and disposition of patient. The person in-charged in the AMMC is a Physician in communication with the physician in each TMC.
Carmelita, when the SYSTEM was being proposed, Abrenians SUGGESTED that the APH must be named in HONOR of Don Quintin Paredes or perhaps named after “SILANG”.
Naming the Hospital Paredes or Silang only memorialized one person… But my proposal is AMMC memorializing people who are for the betterment of the entire province… like Dr. Gaspar Baula , Dr Victor Bobila, Dr. Patronilio Seares , Dr. Luis F. Bersamin,Sr. who were physicians in the province… These were Abrenians worthy of being Memorilized. These were people of the past.
But how about people of the present… can they be memorized . .your guess is as good as mine!!!

If they ignore it for reasons of which they themselves were used to it, maybe you could try the governor or congresswoman to prioritize this as their urgent project because it’s really very important in saving precious life. However doc try to salivate this: not all human being is gifted to have a kind and helpful heart like you. They don’t care others’ immediate needs because selfishness or greed is their nature where they are ever ready only of doing what’s good for themselves and their close relatives or friends. Therefore, as your last resort, you could try Ms. Pura Sumangil, the head of an NGO for human rights, good governance, etc. to fight or convince the heads of the different possible sources of funds needed for the realization of your good intention, doctor.

Do you know why the hospital is as you say exceeding its limit in bed capacity? I think the oroblem is not really the bulk of oatients but rather the number of admissons exceed that of discharges. Why? Matagal gumaling ang oasyente kaya naiipon ang pasyente. Why? It is because of the filthy condition of the hospital. Instead of getting well, the patients get worse because of the dirtiness of the surroundings of the hospital. Instead of patients stay in the hospital for only 2 to 3 days they stay for one week, that answers your claim that the hosital is exceeding its bed capacity. I don’t blame the doctors but the administrators.
One more point, we’ve noticed that most nurses appointed by takit are from penaruby oh oenarubia. Surpassing their seniors in being placed as permanent. Why from oenarubia? Penaruby nga hahahahahaha

Ms. Sargeants Wife, i have great interest as to the location of the emergency room and its ENTRANCE. You see, i have seen many hospitals here in the US and have drawn a conclusion as to what hospital makes the best “functional unit”.
CONSIDER THE FACT THAT the different DEPARTMENTS must be strategically located in the HOSP to function efficiently.
The different Departments are….
1. EMERGENCY ROOM DEPT.
2. SURGERY ROOM DEPT.
3. DIAGNOSTIC DEPT> >>X-RAY, MRI, ULTRA SOUND, CAT SCAN
4. ADMINISTRATIVE DEPT. >>>INCLUDING RECORD DEPT.
5. OBSTETRIC- GYNECOLOGICAL FLOOR
6. SURGERY>>> MALE and FEMALE SECTION
7. MEDICAL>>>> MALE and FEMALE SECTION
8. PATIENT’S FAMILY WAITING ROOM.
9. PRIVATE PATIENTS ROOMS… these are patients covered by insurances AND people who can afford TO PAY from private funds.
IF YOU’RE A NURSE. your knowledge of HOSPITALS, perhaps you can me on this.

There should be a guarantee from the certifying individuals/body/team that they will be liable for prosecution when it will be damaged again by “arbis”. Were they able to determine that structural damage occurred because of inferior standard classification of building materials were used but their costs as reported to their financial report says otherwise or will not jibe? If not, what was the cause then? Is it a sign of bad karma or warning that the hospital is not really safe to use because it is built with rumored corruption? Please enlighten us…..thank you….

The question is how Mayor Eduarte got the contract, he is the nephew-in-law of takit. There is a glaring conflict of interest, takit being the approving official of the contract. Now that there are defects that surfaced due to rains only. Takit will surely cover it up because he will be dragged too if an investigation of a larger scale will be done.

I have not heard that the NEW HOSPITAL was opened to the public…. If I missed the ” Opening” I’m sorrry.
Folks, Carmelita is right…. once the Hospital is opened to the public, it remains opened unless a part of it is being renovated or constuction is going on, a situation that hinders new large – size equipments (like MRI machine) being installed, a part of the Hospital has to be closed to the public. But still, there is a ‘passageway’ for normal flow of the trafic or paients to use in case of Emergency!!!

Dr tuason is very right in his stateents regarding a good hospital. The placement of the emergency room is vital, its access to the operating room, delivery room is essential. Even the proximity of the diagnostic departments like the laboratory and radiologic rooms to the er is vital. The wards may it be charity or pay should also be placed strategically. So is the pharmacy. And mind you that we should not have charity wards anymore because most people are benefiviaries of philhealth sponsored by lgus and 4Ps. So it must be called philhealth wards.
With regards to doctors asking for profesional fees, this I have to say. There are classifications of doctors in the hospital. There are resident physicians, consultants and visiting physicians. Resident physicIans should not charge professional fees because they are not entitled to it. They only get their salaries and their other benefits but not professional fees. They are not even allowed to admit patients at the pay ward. Only vonsultants and visting physicians are allowed to charge professional fees provided that their patients are in the pay wards otherwise they should not charge professional fees.
What we need is a good administrator and polital will amongst our leaders for us to improve on our health care system. We do not need madam X meddling in this because she has nopersonality to meddle even if she is the wife of the governor. Let the administrator of the hosoital do their thing.

1. RESIDENT PHYSICIAN… this is one step above an “INTERN” employed by the hospital receiving a minimal ‘stipent’, PLUS free a) sleeping room accumodation b) food at The hopsital cafeteria c) luandry of unifoms.
THESE PHYSICIANS ARE NEWLY GRADUATES FROM MEDICAL SCHOOL WHO WISHES TO ADVANCE HIS MEDICAL KNOWLEDGE.
DUTY:… He is asigned to an ATTENDING or a VISITING PHYSICIAN who is obligated to teach the INTERN OR RESIDENT as they make their daily rounds in seeing patients. They do not receive ‘fee’ for service.

1 medical officer 3 – these are the new and unpromoted doctors being paid by the provincial government. Slary grade 18.
2. Medical officer 4. Salary grade 22.
3. Medical specialists ranging from 1 to 4.
I do not know the number of doctors occupying these positions. But I know for a fact that some supposed to be consultants or medical spevialists meaning they have finished their residency training and passed their respective boards are occupying either medical officer 3 or 4.
Med officer 3 and 4 are not supposed to collect fee for service but because some of them are graduates of their specialties they are allowed to collect their profesional fees from patients in the pay ward.
The key here is that only patients in the pay ward are to pay professional fees. But I was informed by some patients are being asked to pay profesional fees even if they are in the charity wards.
The solution I suggest for these problem are the following:
1. Update the plantilla positions for the hosital. This will create positions for the doctors who deserve to be med specialist.
2. Implement the salary standardization law for doctors. I mean up to the 4th trance.
Visiting physicians are doctors not employed by the province but were given permission to admit their patients in the hspital.
To alleviate the lack of doctors in the hosital they should make the hospital an open hospital. Meaning any doctor could admit his or her oatient in the hospital. This will lessen the workload of the doctors and more income for the hspital.

With this arrangement… an A.com postings in 2009 listed several anomalies, namely:
1. P HYSICIAN CHARGING FEE FOR SERVICE.
2. PHYSICIAN DIRECTING PATIENT TO BUY HIS/HER MEDICINES AT A PHARMACY OWNED BY THE PHYSICIAN.
3. CONSULTANTS OVER-CHARGING.
In the US we have the so callled “COUNTY HOSPITAL” which is the equivalent of a provincial Hospital, admits all patients… insured, paying, or indigent (poor)….based on FINANCIAL STATUS.
The Admitting Office Department personnel determines the financial status of the patient.
There are physicians having a private practice in town… some would like to see more patients so as to increase their income… by applying to the Hospital, submit their credentials to the Hospital Adminstrator for a privilege to admit his patients to the Hospital to treat, himself Physician-in-Charge. … these are called ” ATTENDING PHYSICIANS”. THEY ARE NOT EMPLOYED BY THE HOSPITAL OR GOVERNMENT.

RESIDENT PHYSICIAN DO NOT CHARGE PATIENT FOR PROFFESION SERVICE FEE. BECAUSE, HE / SHE IS EMPLOYED B Y THE HOSPITAL. SHE/HE GETS MONEY (STIPEND), OTHER BENEFITS FREE FOOD-LODGING,LAUNDRY FOR UNIFORMS.
VISITING PHYSICIAN…. Is not employed by the HOSPITAL.. but can admit patient to the hospital to treat. HE CAN CHARGE the patient any amount he wanted to.

This is how the hospital is operating. The reported anomalies in 2009 is stll what is happening at the hospital. There are drs who charge professional fees even if the the patients are in the charity ward. They send their patients to their pharmacies. Their prescription could only be read by their store keepers. Sad to say some drs are also overcharging even if the patients have their philhealth.
There is an office that classify patients. But unfortunately even when classified as indigents, they still pay a ertain amount. But if yiu know somebody in the hospital the and only then that you pay nothing.
With the new program of dswd and doh providing philhealth cards to indigents, the plight of our less fortunate kababayans is somehow alleviated. But unluckily again, they are being charged of the “excess”. This should not be as stated by the AVP of philhealth.
Sometimes we could not blame the doctors because they have to augment their salaries because the province has not yet implemented the full standardization of salaries.
Sometimes I don’t even blame thes doctors in sending their patients to buy their medicines outside, because in the first place, there is n supply of medicines in the hspital pharmacy.
The hospital should be aplenty of medical supplies and medicines for it to function well. We need political will to see to it that our health care system will function well.

1. Physicians charging MONEY for proffessional fee from patients in the CHARITY WARD.
2. Physicians instructing their patients to buy the written prescribed drug at a PHARMACY probably owned
>> by the prescribing doctor to the manager of the drug-store usually his wife who is a pharmacist… or a
>> person a pharmacist who is employed if the doctor’s wife is not a pharmacist.
3, Physician charges certain amount from classified indigent patient.
4. If patient is acquainted with someone connected with the Hospital… patient pay nothing!!!
5. HOSPITAL PHARMACY is not adequately supplied with medicines.

you are absolutely right in your posting.
let me just clarify my point in one of my postings regarding the plantilla positions and the doctors who charge their patients.
all doctors occupying the plantilla positions in the hospital are being paid by the provincial government may it be the doctor is a medical officer 3 or 4, or medical specialists 1-4. they all receive salaries from the provincial government..
in the normal setting and circumstance, the medical officers 3 and 4 should not be charging professional fees, bacause these positions are supposed to be occupied by the doctors who have not gone to residency training and have not finished it yet. but what is happening at the hospital is that because of lack of plantilla positions for medical specialists, t the doctors who have finished their residency training and had passed their respective specialty boards are appointed to the either medical officer 3 or 4. here arises the problem of who could charge their patients in the pay ward.
the admissions in the pay ward are the responsibilities of the med specialists or what they call at the abra provincial hospital as “consultants”.but because there are specialists who occupy plantilla positions as either med officer 3 or 4, they are allowed to admit patients in the pay ward and charge them for their professional fees.
having the opportunity to have studied public health and having had the chance to intermingle with the doctors, i know their woes and concern. that is why in my previous posting i suggested several solutions.
dr. leo, the set-up in the states is very much different to the set-up here especially when it comes to the hospital setting. where have you seen a secondary hospital without a medical director. i dont think that a plantilla position for medical director is even present in the plantilla of the provincial government.
while there are 3 provincial health officers in the province, 1 PHO II, now being held by dra berona, and two PHO I, being held by drs pascua and enerva, there is no official designation as to whois in-chrage of the hospital. i dont even know the designation of dr gaza who everybody knows as the big boss at the provincial hospital. i know for a fact that dr gaza was borrowed from DOH but i dont know for what.my proposal is that a position of medical director be created and filled, so that we know who to talk to or even blame when there will be anomalies found in the hospital. we need to have somebody responsible solely for the hospital.

Mario, you are confronted with a hugh problem. I like your solution for Abra Provincial Hospital but the main problem is the DEPT OF HEALTH. The whole Plantilla of the HOSPITAL doctors positioning has been in existence for so many years. CHANGES HAS TO BE MADE IN THE DEPT. OF HEALTH!!!

it is no longer the responsibility of the department of health when it comes to plantilla positions. remember that in the 1990s, the provincial hospital and all government hospital including the rhus were devolved to the province and the respective LGUs. the devolution paved the way for the governor and the province to take control of the hospitals and the mayors taking control of the RHUs. along with it is the funding of the hospitals were also devolved to the province, that is why the IRA or the internal revenue allotment of the province grew, because the funding for the hospitals no longer pass thru the department of health but goes directly to the coffers of the province.
you are right in saying that these plantilla positions were the ones used before the devolution by DOH. but because of the increase in population and increase of clients, this set-up of plantilla positions is already outdated. It is now within the inherent power of the province thru the governor and the provincial board to alleviate this problem of lacking plaantilla positions for the hospital. the provincial board (sangguniang Panlalawigan) can create positions in addition to the present plantilla positions with the approval of the governor.
the set-up at BGH is so much different from that of the provincial hospital. remember doc that BGH is a tertiary hospital and a training center at that but APH is a secondary hospital and a non-training hospital. I am familiar with the set-up of training hospitals and i know the ins and outs of the training hospitals.In the abra provincial hospital everybody who finished their respective specialties even if they have not passed their specialty boards are called “consultant” may it be that their official plantilla position is medical officer 3 or 4 or medical specialists. Let me reiterate that with the size of the APH, it should have a medical director, something that it has none.
drs berona, pascua and enerva are indeed in government service but mind you doctor, they also have their private practice at the provincial hospital and other hospitals where they are accredited.
you see doctor, if only takit and his advisers know the impact of creating a very good health care system, then he would not have to worry about these problems that are arising now. what we need is political will.
another problem here is that he is asking his wife, ruby, about hospital matters. taking for granted that she is a nurse or reached second year medicine proper, this does not mean that she has the expertise to know the ins and outs of the administration of the hospital. I wonder what dr. gaza is doing. I know dr gaza having graduated from a prestigious school of medicine in UST and having a masters degree in hospital administration should be knowledgable enough to run the hospital. But it seems to me that he could not assert his presence or he is just hearing and doing what takit wants or takit’s wife wants. you see doctor, what we need are experts not amateurs.

IN OUR CITY, anyone physician (new graduates or old) can come in and put up his/her private pracitce in the CITY.
Very open these physicians apply for HOSPITAL previleges so as to make use of the HOSPITAL facilities.
PHYSICIANS considered as specialists would apply and present their training credentials to the DEPT. HEAD
for approval.
NONE OF THESE PHYSICIANS ARE EMPLOYED BY THE HOSPITAL or GOVERNMENT.
ALL PHYSICIANS ARE CALLED PRIVATE PRACTITIONERS.

CASE/ EXAMPLE….
An INDIGENT (CHARITY PATIENT) or PAY- PATIENT comes in with Fever, Diarrhea and Abdominal Pain.
The patient shall be assigned to GENERALIST (DR. LUNES) Initially, the patient is seen by the INTERN working with DR. LUNES… while the GENERALIST is enroute to the Hospital from his/her office.
DR. LUNES concurs with the diagnosis of his INTERN>>.the patient may have “ACUTE APPENDICITIS” .
The INTERN now calls the INTERN who is working with SPECIALIST GENERAL SURGEON assigned for that day (MONDAY) to see the patient.. The GENERAL SURGEON decides to OPERATE.
The outcome>>>> acute appendicitis….. THE GENEREAL SURGEON gets paid for services on PAY-PATIENT.
on CHARITY-PATIENT…services is FREE. The patient remaind under the care of the Surgeon until he is dismissed.
IF the case is a medical case… that is diagnosed as ‘GASTRO-ENTERITIS’. the patient remains under the medical care of DR. LUNES untl patient gets well.
Now MARIO, your impression on the differences between the Provincial Hospital and the County Hospital..
WHICH ‘PLANTIILA’ WORKS BETTER….that is less ANOMALIIES.

Very cooperative people and I respect them so much for their kindness and appreciation for all that I did.
I interviewed the GM many few times in person with the BOD’s and all of the abeco staffs. I find all of them very cooperative and friendly. Nervous? yes they were and very careful. coming up…..LET’S SET THE RECORD STRAIGHT!!!
